What Is the Rural Health Transformation Program (RHTP)?
The Rural Health Transformation Program (RHTP) is a federal program that provides funding to states to support changes in rural health care. CMS describes $50 billion in funding across fiscal years 2026–2030; a CMS award to a state does not itself create an application for a local provider, vendor, or community organization.
How the program is structured
CMS says the program was authorized by Section 71401 of Public Law 119-21 and allocates $50 billion to approved states across fiscal years 2026 through 2030, with $10 billion available each year. Half of the funding is distributed equally among approved states. CMS allocates the other half using factors specified in the federal Notice of Funding Opportunity, including rural population and characteristics of a state's rural health system.
Only the 50 U.S. states are eligible for these federal awards. CMS announced awards to all 50 states in December 2025. That federal award is the beginning of state implementation, not a nationwide application portal for providers or vendors.
What states may use RHTP funds for
States must select three or more approved uses. The federal program page describes a broad set of options, including prevention and chronic-disease work, provider payments, workforce recruitment and retention, technology-enabled care, cybersecurity and information technology, behavioral-health and substance-use services, right-sizing care delivery, and innovative payment models.
Those categories are useful signals of what a state may prioritize. They are not a promise that every state will issue a grant, an RFP, or a subcontract in each category. A state may use a mix of internal implementation, provider payments, contracts, grants, partnerships, or other approaches.
Federal award versus state opportunity
This distinction is the part most worth getting right. A CMS award announcement confirms that a state has federal funding and a strategy to carry out. It does not tell a local organization that it can apply, what it can apply for, or when a response is due.
The same is true of a state project narrative, abstract, webinar, FAQ, meeting notice, or general RHTP page. Those materials can be excellent context. They do not become a solicitation merely because they mention funding. An actionable item is the official document or portal notice that identifies an issuing agency, scope, response instructions, and a deadline or procurement schedule.
